minutes to complete.

Send your forms to our Oakville office, with a payment
of $20.00, or pay by paypal from our homepage. This
fee will be discounted from your first session.
• Our
office will contact you within 3 business days of receiving
your information.
• Your
session will be approximately 1 hour long.
• If
you wish to proceed with a Complete Nutritional Health
Assessment, discounts are available, if you have already
done an iridology session

Although optional, and totally at your discretion, to
ensure the success of your program, follow-up appointments
are recommended at weeks 3, 5 and 8, after which time
most clients return seasonally, to re-vamp their program.
